There are a great many forms that can be found on the UK government website regarding Court of Protection applications, assessments and statements.
The abbreviations can be unclear as to the purpose of the form for those who do not use them on a regular basis. However, the government provides useful summary descriptions of each form to direct the user to which item is most applicable to a given situation.
These include, but are not limited to:
- Form COP DLA: Deprivation of Liberty application form – urgent
- COP1/COP1A/COP1B/COPC/COP1D/COP4: Five different forms used to apply to make decisions on someone’s behalf
- COP7: An application to object to the registration of a Lasting Power of Attorney (LPA)
